In addition to the products and services offered on Ottawa Derm Centre’s website, Ottawa Derm Centre also offers an online service to enable users to register for a loyalty program (“ODC HEALTHCOIN REWARDS Registration Service”).
In order to become a user of Ottawa Derm Centre’s website and/or to become a registrant of the ODC HEALTHCOIN REWARDS Registration Service, you must read and accept all of these terms and conditions and Ottawa Derm Centre’s Privacy Policy
In the event of any inconsistency between Ottawa Derm Centre’s Privacy Policy and the Terms of Service, the Terms of Service shall prevail. Healthcoin Loyalty Program
Ottawa Derm Centre offers a loyalty program that rewards clients with ODC HEALTHCOIN REWARDS to be applied towards purchases of products and services at Ottawa Derm Centre, subject to exclusions. Such loyalty program is subject to its own terms and conditions, which are incorporated by reference and are available at https://healthcoinrewards.ottawadermcentre.com
User Eligibility
ODC HEALTHCOIN REWARDS Registration Service is subject to qualification and is not available to minors under the age of 18 or to any users suspended or removed from the system by Ottawa Derm Centre for any reason. If you do not qualify, you may not use ODC HEALTHCOIN REWARDS Registration Service.
Use of Service
Your use of the ODC HEALTHCOIN REWARDS Registration Service and website is governed by the Terms of Service. ODC may refuse service with or without prior notice to any user for any or no reason.
You are responsible for maintaining the confidentiality of your password and account (if applicable) and are fully responsible for all activities that occur under your password or account with or without your knowledge. You are also responsible for the input of all your information. Ottawa Derm Centre cannot and will not be liable for any loss or damage to you or any third party arising from your failure to comply with this section.
Termination
You agree that Ottawa Derm Centre may, with or without cause, terminate or restrict access to your ODC HEALTHCOIN REWARDS Registration Service account (if applicable) and/or access to the ODC HEALTHCOIN REWARDS Registration Service. User Conduct
You understand and agree not to:

- Falsely state, impersonate, or otherwise misrepresent your identity, including but not limited to the use of a pseudonym or misrepresenting your affiliations with a person or entity, past or present.

- Upload, email, transmit or otherwise make available any material that contains software viruses or any other computer code, files or programs designed to interrupt, destroy, or limit the functionality of any computer software or hardware or telecommunications equipment.
- Forge headers or otherwise manipulate identifiers in order to disguise the origin of any communication transmitted through the ODC HEALTHCOIN REWARDS Registration Service.
- Post content in fields that aren’t intended for that content. Example: Putting an address in a name field.
- Interfere with or disrupt ODC HEALTHCOIN REWARDS Registration Service or servers or networks connected to ODC HEALTHCOIN REWARDS Registration Service and website, or disobey any requirements, procedures, policies, or regulations of networks connected to ODC HEALTHCOIN REWARDS Registration Service and Ottawa Derm Centre’s website.

If Ottawa Derm Centre’s internet services provider, for any reason, is unable to bill or collect any amounts according to the user/registrant’s requested payment method, then the user/registrant will remain responsible for all such amounts. If the user/registrant fails to pay applicable fees when due, then Ottawa Derm Centre and/or its internet services provider, in its sole and absolute discretion, may deny providing to the user the unpaid product or service and/or may pursue such remedies as are available under applicable law for the collection of delinquent fees.
Indemnification

YOU EXPRESSLY UNDERSTAND AND AGREE THAT OTTAWA DERM CENTRE SHALL NOT BE LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL OR EXEMPLARY DAMAGES, INCLUDING BUT NOT LIMITED TO DAMAGES FOR LOSS OF PROFITS, GOODWILL, USE, DATA OR OTHER INTANGIBLE LOSSES (EVEN IF OTTAWA DERM CENTRE HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES) RESULTING FROM YOUR USE OR INABILITY TO USE THE OTTAWA DERM CENTRE’S WEBSITE OR THE ODC HEALTHCOIN REWARDS REGISTRATION SERVICE OR WEBSITE.

Ottawa Derm Centre’s logo, ODC HEALTHCOIN REWARDS and other Ottawa Derm Centre trade names are trademarks (pending or registered or through common law rights) of Ottawa Derm Centre. You agree not to display or use these trademarks in any manner without Ottawa Derm Centre’s prior written consent, which consent may be unreasonably withheld. The section titles of this Terms of Service are displayed for convenience only and have no legal effect. Governing Law
The Terms of Service and the Privacy Policy, and all transactions between you and Ottawa Derm Centre, including those through ODC HEALTHCOIN REWARDS Registration Service, will be governed by and construed in accordance with the laws of the Province of Ontario (without regard to conflict of laws principles).
Entire Agreement
These Terms of Service and Ottawa Derm Centre’s Privacy Policy constitute the entire agreements between you and Ottawa Derm Centre and govern your use of Ottawa Derm Centre’s HealthCoin Rewards Registration Service and website, superseding any prior agreements between you and Ottawa Derm Centre regarding the subject matter.
